Here's what I have:
1. The name of your provider? Time Warner i.e. Road Runner
2. Type of service it is? Either Satellite, Cable, Broadband, Dial-up etc. Cable
3. The name of your providers package? None when I got it 4 years ago
4. What country you live in? Akron, Ohio USA
5. What ports are blocked if any? None yet, but port 21 (FTP) is choked back to 52KBPS up speed
6. Any other relevant info? 6.5 mb down 800 KBps up.
